Creating form templates

Form builder
Forms are created by combining and customising blocks to insert all the fields required to capture necessary information for a request.
Form elements
Below is a list of descriptions for the blocks used to create form templates.

Short text | Text field (60 character limit) | Email field | |

Dropdown | Dropdown list (single answer only) | Phone | Phone number field |

Building templates
The following tools and options are available for building and customising form templates.
Insert a block: Click on a block under "Form elements" to insert it into the builder. | |
Delete a block: Click the bin icon on a block to delete it. | |
Make field mandatory: Check the box next to "Mandatory" on a block. A user will not be able to submit the form until mandatory fields are completed. | |
Re-order blocks: clicking anywhere on a block enables it to be dragged and dropped in another position in the workflow. | |
Edit text: click the pencil icon next to a field to rename it or add a description. | |
Add option: creating options for users to choose from is required when using the single or multiple choice blocks. Click "Add option" and then "Add field" to insert any choices. Click "Submit" to save the options. |
Managing form templates
Edit template
- Click on the name of the template you want to edit.
- This re-opens the form builder and allows any changes to be made.
Rename template
- Click on the checkbox of the template you want to edit.
- Click the pencil / edit icon.
- Enter a new name and click "Edit" to confirm.
Duplicate template
- Click on the checkbox of the template you want to duplicate.
- Click the copy icon.
- A duplicate form will be created and appear at the bottom of the list, named "copy".
Delete template
- Click on the checkbox next to the template you want to remove.
- Click the bin icon and then "Delete" to confirm the removal.
